Consent to physician-ordered testing

This Consent describes, and asks you to agree to, the process by which a physician affiliated with the Telehealth Provider may order laboratory testing for you as part of your Insyght Membership. Please read it carefully before proceeding.

1. How your testing is ordered

After you complete intake and provide your health history, a licensed physician or other qualified clinician affiliated with the Telehealth Provider will review that information. Based on the clinician's own independent clinical judgment, not a decision made by Insyght, the clinician may order a laboratory panel appropriate to assess your health. Insyght does not determine whether testing is medically appropriate for you, and does not select, approve, or override the clinician's order.

2. What this means for you

• The laboratory test you receive is ordered by a physician who has reviewed your information, not something you are ordering for yourself as a consumer product.
• The clinician may determine that no testing, or different testing than you expected, is appropriate for you. If so, you will be informed, and no unordered test will be run.
• Your specimen will be collected at a designated patient service center or, where available, at home by a certified phlebotomist, and processed by Insyght's laboratory partner for your region (e.g., Quest, or BioReference in New York and New Jersey, through Junction).
• Your results will be reviewed and interpreted, in part, through the Insyght Platform's health-scoring methodology, and made available to you through your Membership.
• A licensed clinician reviews and approves your results before they are released to you; nothing from your results is shown to you before that approval.
• Your results may also be reviewed by the ordering clinician as part of your care with the Telehealth Provider. Your results and related clinical records are maintained as the Telehealth Provider's medical records; Insyght's access to them is limited to what is necessary to generate your health scores and reports and to support your Membership.
• The Telehealth Provider and its clinicians decide, in their sole discretion, whether to accept your request for evaluation, and may determine that a request is not suitable for telehealth or that no testing is appropriate.

3. What this consent covers

By accepting this Consent, you agree to:

• The formation of a physician-patient relationship with the Telehealth Provider's clinician for the limited purpose of this order
• The clinician ordering laboratory testing based on their independent clinical judgment
• Collection of your specimen at a designated patient service center or, where available, at home
• Processing of your specimen and results by Insyght's laboratory partner
• Interpretation of your results through the Insyght Platform, and sharing of your results between Insyght and the Telehealth Provider as necessary to provide your Membership and your care
• Your financial responsibility for the amounts described in the Financial Disclosure provided to you separately

4. Limitations of testing

Laboratory testing has limitations. A result within a reference range does not guarantee the absence of a health condition, and a result outside a reference range does not by itself establish a diagnosis. Test results should be considered together with your overall clinical picture, as assessed by a licensed clinician.

5. Your right to decline

You may decline testing at any time. Declining testing does not, by itself, end your Membership, though it may limit the health scores and reports Insyght can generate for you. Declining testing does not affect any unrelated services you separately receive.

6. Your acknowledgment

By accepting this Consent, you acknowledge that you have read and understood it, had the opportunity to ask questions, and voluntarily consent to physician-ordered testing on the terms described above. You accept this Consent electronically as part of your intake, recorded with your member name and the date of acceptance.
